Pre-Registration Forms:
Register Online
Register for session(s), extra programs, busing, trips and leadership programs.
Paper Registration Form
Download, fill out, SIGN and either mail or fax to camp.
Post-Registration Forms:
After we receive your registration, we will mail out a confirmation package with all the following forms. If you have not received it within 3 weeks please call the camp office to confirm receipt of your registration.
Resident Camp Guide
Loaded with valuable information, camp procedures and a helpful checklist to get your camper ready.
Health History Form*
This completed form requires a parent/guardian signature for camper to attend camp. A physical or doctor's signature is not required.
Letter to Counselor*
This form will help our directors and your camper's counselor get to know your camper before his/her arrival at camp.
Camper Waiver/Release Form*
Fill out this form so our staff knows when your child will be leaving and who will be authorized to pick them up.
Camper Ethnic Form*
This is an anonymous questionnaire with the sole purpose of providing information to the organizations that make it possible to maintain a non-profit camp for boys and girls.
* These four forms are due back at the camp office BEFORE May 3rd, 2010.
No forms, no camp. :)
Bus/Car Transportation schedule
Check bus stops and times for your camper. This form also goes through the check-in and check-out procedures.
Trading Post Form
Each camper must have a Trading Post account set up prior to attending camp. Access your online account or use this form to put money in your camper's account.
If you have a question, no matter how small, please call anytime. We want you and your camper to be ready for Camp! :)
