Contains rates, dates and general Day Camp info.

Day Camp Enrollment Form
General enrollment form with contact information for parents.

Health History and Emergency Care Plan Form
This completed form requires a parent/guardian signature for camper to attend camp. A physician or doctor’s signature is not required.

Immunization Record Form
An up-to-date immunization record is required for attendance. You can have your doctor’s office fax the record or fill in each box with specific dates.

Camp Jorn YMCA Day Camp is located on the Camp Jorn property on Rest Lake in Manitowish Waters, Wisconsin. This program is offered for 11 weeks during the summer. It provides safe, affordable child care for working families as well as exciting camp experiences and peer social interactions for visitors to northern WI. Age appropriate activities which include swimming, canoeing, kayaking, arts and crafts, nature hikes, horseback riding, day trips, sports and much more are taught and supervised by trained, enthusiastic summer day camp counselors. Children can be registered for part time, full time, part of the summer or for the entire summer. This program is available for children in two age groups:
