Below are downloadable and printable forms required for Day Camp attendance.

Day Camp Flyer/Registration form 

Contains rates, dates and general Day Camp info.

Day Camp Enrollment Form

General enrollment form with contact information for parents.

Health History and Emergency Care Plan form

This completed form requires a parent/guardian signature for camper to attend camp.  A physician or doctor's signature is not required.

Immunization Record Form

An up-to-date immunization record is required for attendance.  You can have your doctor's office fax the record or fill in each box with specific dates.
