Child Care Registration Form
Required for all children attending Child Care.

Health History / Emergency Care
Requires a parent/guardian signature for child to attend.

Child Health Report
Requires a health provider signature for child to attend.

Child Care Immunization Record
An up-to-date immunization record is required for attendance. You can have your doctor’s office fax the record or fill in each box with specific dates.

Arrival/Release Agreement
When your child will arrive at the center from school or other activities, or depart from the center to go to school, and the child will not be accompanied by a parent.

Intake for Child Under Two
Required for those children under two years old.

Child Care

Camp Jorn YMCA offers a full day, year round quality child care program using a play-based curriculum which allows each child to grow at their own pace in the areas of social, emotional, physical and academic development. Warm, caring, educated staff provide care in a casual, relaxed setting where the needs and interests of both child and parent are recognized.

Summer child care available by the day, week or month. Must be preregistered. No drop-ins. Call or email for more information.

Check out our Day Camp program, available for two age groups: 5 to 6 year olds and 7 to 12 year olds.
